Role Overview

The Network Outreach Coordinator facilitates referrals and client assignments for Enhanced Care Management between Full Circle Health Network providers and Medi-Cal managed care plans. Uses motivational interviewing techniques to encourage clients to consent to ECM participation and ensures consenting clients are referred to the best Full Circle provider for their whole-person needs.

Duties & Responsibilities

  • Conduct outreach and engagement (phone calls, mailers, field visits) to facilitate ECM enrollment
  • Serve as day-to-day contact for managed care plans and network providers on referral and case status
  • Develop a deep understanding of the Full Circle provider network to support appropriate referrals
  • Monitor client assignments and complete tasks within required timeframes
  • Engage clients across multiple modalities: text, email, telephone, and in-person
  • Escalate issues to the Network Operations Director; provide input into quality improvement
  • Complete all documentation in the required electronic system within required timeframes
  • Attend meetings with managed care plans and providers; attend assigned trainings

Minimum Requirements

  • At least 3 years of relevant work experience
  • High school diploma or GED required
  • Long-time community resident with knowledge of local resources
  • Ability to work flexible hours including occasional evenings/weekends

Preferred Qualifications

  • Multilingual capabilities preferred
  • Prior experience as a Community Health Worker, Peer Support Specialist, or Medical Assistant

Compensation

Part-Time Starting Rate: $19.00 – $24.00 per hour Rate is commensurate with experience.
Full-Time Salary: $44,000 – $56,000 per year
Rates are competitive with LA County and Orange County market standards for ECM Network Outreach Coordinator roles. Pay increases are available based on performance and experience.

Pre-Employment Requirements

All candidates must meet the following requirements prior to their start date:

  • TB Test (Tuberculosis Screening) — A current negative TB test result is required. This must be completed and cleared before your first day.
  • Background Check — All candidates must successfully pass a DOJ/FBI LiveScan fingerprint background check. A criminal history does not automatically disqualify you; records are reviewed on a case-by-case basis.

Please note: All positions require candidates to pass a TB (tuberculosis) test prior to starting employment.
