Position Summary
Under the direction of the ECM Program Manager/Supervisor and/or Clinical Consultant, the Lead Care Manager (LCM) provides direct service and coordinates care management as part of a “Care Team” for the Enhanced Care Management Program. The LCM oversees specific cases, coordinates health care benefits, provides education, and facilitates client access to care in a timely, cost-effective manner. The LCM collaborates with clients, caregivers/family, and other providers to promote wellness, recovery, independence, resilience, and empowerment — serving as an advocate for clients and a liaison with community health and social service providers.
Reporting & Work Setting
- Reports to: ECM Program Manager/Supervisor and Clinical Consultant
- Location: Hybrid — office and field (home visits, community-based locations, telehealth)
- Hours: Flexible, including occasional pre/post business-hours and weekend coverage as needed by members
- Service area: Los Angeles, Orange, Riverside, and San Bernardino Counties
Key Duties & Responsibilities
- Assess client needs across physical health, mental health, substance use, oral health, trauma-informed care, social supports, housing, vocational/employment, and wellness
- Develop and oversee implementation of the Individual Care Plan / Health Action Plan
- Deliver services where the client lives, seeks care, or finds most accessible (office, telehealth, or field-based)
- Connect clients to needed social services and community supports
- With permission, coordinate and advocate with the client’s other health care providers (e.g., PCP)
- Apply evidence-based practices: Motivational Interviewing, Harm Reduction, Trauma-Informed Care
- Collaborate with hospital staff on Transitional Care Planning
- Conduct outreach and engagement (calls, mailers, field visits) to enroll members in ECM
- Accompany clients to office visits as needed
- Evaluate progress and update goals; provide mental health promotion and arrange transportation
- Complete all documentation within required timeframes; attend weekly team meetings, supervision, and assigned trainings
Knowledge, Skills & Abilities
- Interpersonal savvy — builds trust and strong relationships quickly
- Comfortable conducting home visits and community outreach
- Experience with EMR/EHR or care management software (or ability to learn quickly)
- Strong organizational skills; able to manage a personal schedule and meet deadlines
- Able to work independently and as part of a team, including virtually
- Excellent oral and written communication; sound professional judgment
- Active listening and genuine compassion for others
- Knowledge of local community resources
- Cultural competency working with diverse client populations
Minimum Requirements
- At least 3 years of relevant work experience
- High school diploma or GED required
- Long-time resident of the community with good knowledge of local resources
- Ability to complete the training program and ongoing educational requirements
- Ability to work flexible hours, including occasional night/weekend work
Preferred Qualifications
- Multilingual capabilities (preferred, not required)
- Prior experience as a Community Health Worker, Peer Support Specialist, Medical Assistant, or similar role
Background Check & Compliance
Per the Full Circle Health Network Provider Agreement, any team member with direct patient contact or access to patient records must complete a criminal background check within three months of start date, covering the past three years of work history in every state worked.

Compensation
Part-Time Starting Rate: $20.00 – $25.00 per hour Rate is commensurate with experience.
Full-Time Salary: $48,000 – $58,000 per year
Rates are competitive with LA County and Orange County market standards for ECM Lead Care Manager roles. Pay increases are available based on performance and experience.
Pre-Employment Requirements
All candidates must meet the following requirements prior to their start date:
- TB Test (Tuberculosis Screening) — A current negative TB test result is required. This must be completed and cleared before your first day.
- Background Check — All candidates must successfully pass a DOJ/FBI LiveScan fingerprint background check. A criminal history does not automatically disqualify you; records are reviewed on a case-by-case basis.
